New Patient Registration Form
We have made the whole process as simple and as hassle-free as possible. You, the patient, decide which service you would like to take advantage of. Would you like us to:
- Collect your prescription from your GP surgery
- Deliver your medication to you
- Have your medication available for you to collect from our pharmacy
We have decided not to charge our patients for this service. Some pharmacies are charging £60 per year, per patient. Our service is free, it won't cost you or the NHS a penny more and it could save you lots of time and effort. All you need to do is download and print the registration form, print and submit to us to take advantage of our services.
